What kind of services are we Certified to offer?

H.O.P.E. received the highest level of review from Home & Community Based Waiver Services in February, 2005 ~ a 3 year certification with EXCELLENCE!  We are also contracted with Polk County Health Services as a PCHS Network Provider. The services we offer are:

Supported Community Living (SCL) Consumer Direct Attendant Care (CDAC) Respite Supported Employment (SE) Transportation

Who qualifies for service with H.O.P.E.?

Children and Adults who have been diagnosed with Mental Retardation and / or Developmental Disabilities (some examples might include: Autism/Asperger's and Cerebral Palsy). Elder Iowans who qualify for the Elderly Waiver or Medicaid funding. Children and Adults who qualify for the Brain Injury Waiver Children and Adults who qualify for the Ill and Handicapped Waiver
